Licensed Vocational Nurse (LVN) – Clinical Assistant
Concho Hearts Hospice:
Concho Hearts Hospice is looking for a compassionate, organized, and dependable Licensed Vocational Nurse (LVN) to join our team as a Clinical Assistant.
This position is a great fit for an LVN who enjoys hospice care but is also interested in supporting the clinical team from an administrative and coordination role. The Clinical Assistant will work closely with our Director of Nursing, nurses, and interdisciplinary team to help keep clinical operations organized, timely, and focused on providing exceptional patient care.
Responsibilities:
- Assist the Director of Clinical Services with day-to-day clinical operations.
- Help coordinate patient care, visits, orders, and clinical follow-up.
- Review charts and documentation for completeness and timely completion.
- Assist with physician orders, certifications, care plans, and other clinical documentation.
- Help prepare clinical information for IDT meetings.
- Follow up with clinical staff regarding outstanding documentation or required corrections.
- Communicate with physicians, facilities, pharmacies, DME providers, patients, and families as needed.
- Assist with admissions, recertifications, discharges, and other clinical processes.
- Help ensure clinical records remain organized and survey-ready.
- Provide LVN patient visits or clinical coverage when needed and within scope of practice.
- Support the clinical leadership team with additional duties as needed.
Qualifications:
- Current Texas LVN license in good standing.
- Hospice experience required.
- Strong organizational and communication skills.
- Comfortable working with electronic medical records and clinical documentation.
- Ability to prioritize multiple responsibilities and follow through on tasks.
- Detail-oriented and dependable.
- Compassionate, professional, and team-focused.
- Knowledge of hospice regulations and documentation requirements is a plus.
