Hospice Account Executive/Community Liaison

Concho Hearts Hospice is seeking an experienced Hospice Account Executive / Community Liaison to join our growing Buffalo team.
This role is ideal for someone who enjoys building strong referral partnerships, educating providers about hospice services, and helping patients and families access compassionate end-of-life care.

What You’ll Do:
- Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities.
- Grow referral sources throughout the Buffalo service area.
- Provide hospice education to healthcare providers and community partners.
- Meet with patients and families to explain hospice services and obtain consents.
- Coordinate closely with the clinical team to ensure seamless admissions.
- Represent Concho Hearts Hospice as a trusted community resource.
What We’re Looking For:
- Minimum 2 years of hospice marketing experience required.
- Proven relationship-building and networking ability.
- Self-motivated with strong organizational skills.
- Comfortable presenting hospice education to providers and community groups.
- Ability to work closely with clinical teams to coordinate patient admissions.
