The decision to begin hospice care can be overwhelming for patients and families. At Concho Hearts Hospice, our Admissions Nurses help make that transition a little easier.
We're looking for a compassionate and confident Registered Nurse (RN) to join our Kilgore team as a Hospice Admissions Nurse.
This is a great opportunity for an RN who enjoys patient interaction, education, clinical assessment, and helping families understand what hospice care can provide.
What You'll Do:
- Complete hospice admission visits in patients' homes, hospitals, facilities, and other care settings.
- Perform comprehensive nursing assessments.
- Educate patients and families about hospice services, medications, equipment, and what to expect.
- Review and complete admission documentation and required consents.
- Collaborate with physicians and the hospice interdisciplinary team.
- Coordinate medications, DME, supplies, and other immediate patient needs.
- Help ensure a smooth transition into hospice services.
- Communicate important patient information to the RN Case Manager and clinical team.
- Complete accurate and timely documentation in the EMR.
- Provide reassurance and compassionate support to patients and families throughout the admission process.
What We're Looking For:
- Active Texas Registered Nurse (RN) license.
- Strong nursing assessment and communication skills.
- Hospice or home health experience preferred.
- Comfortable educating patients and families about end-of-life care.
- Organized and able to manage admission documentation efficiently.
- Compassionate, dependable, and confident working independently.
- Reliable transportation, valid driver's license, and current auto insurance.
- Current CPR certification.
