At Concho Hearts Hospice, our Chaplains provide comfort, encouragement, spiritual guidance, and a listening ear to patients and families during some of life's most meaningful moments.
We're looking for a compassionate and experienced Hospice Chaplain to join our team serving Brazos County and surrounding communities.

What You'll Do
- Provide compassionate spiritual and emotional support to hospice patients and their families
- Complete spiritual assessments and develop individualized spiritual care plans
- Offer prayer, counseling, encouragement, and spiritual guidance based on each patient's personal beliefs and wishes
- Support patients and families experiencing anticipatory grief, loss, and end-of-life concerns
- Respect and support individuals of all faiths, cultures, and spiritual backgrounds
- Collaborate with nurses, social workers, physicians, and other members of the hospice care team
- Participate in interdisciplinary team (IDT) meetings as appropriate
- Coordinate with local clergy, churches, and community spiritual resources when requested
- Maintain accurate and timely documentation in the EMR system
- Provide additional spiritual support during times of crisis or increased patient and family needs
What We're Looking For
- Previous hospice chaplain, pastoral care, ministry, or spiritual counseling experience preferred
- Education or training appropriate for providing spiritual and pastoral care
- Strong listening, communication, and interpersonal skills
- Compassionate and respectful approach to individuals with diverse beliefs and backgrounds
- Ability to provide emotional and spiritual support during difficult situations
- Comfortable working independently while collaborating with an interdisciplinary team
- Strong organizational and documentation skills
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout Brazos County and surrounding communities
