Hospice Account Executive / Community Liaison
Location: Houston, TX
Are you an experienced hospice marketer who thrives on building strong referral relationships and making a difference for patients and families?
At Concho Hearts Hospice, our liaisons are trusted partners in the healthcare community. We’re looking for a motivated professional who is passionate about hospice care and enjoys developing meaningful partnerships with providers throughout the Fort Worth area.
What You’ll Do:
- Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities.
- Grow referral sources throughout the Houston service area.
- Provide hospice education to healthcare providers and community partners.
- Meet with patients and families to explain hospice services and obtain consents.
- Coordinate closely with the clinical team to ensure seamless admissions.
- Represent Concho Hearts Hospice as a trusted community resource.
What We’re Looking For:
- Minimum 2 years of hospice marketing experience required.
- Proven relationship-building and networking ability.
- Self-motivated with strong organizational skills.
- Comfortable presenting hospice education to providers and community groups.
- Ability to work closely with clinical teams to coordinate patient admissions.
