At Concho Hearts Hospice, we're looking for a compassionate Licensed Vocational Nurse (LVN) who enjoys variety. This unique position combines direct patient care with office-based clinical support, allowing you to make a meaningful impact both in the field and behind the scenes.

What You'll Do:
Field Responsibilities:
  • Provide compassionate nursing care to hospice patients in their homes or care facilities.
  • Perform routine nursing visits and assist with admissions as assigned.
  • Monitor patient conditions and communicate changes to the RN Case Manager and physician.
  • Administer medications and treatments as ordered.
  • Educate patients and families on disease progression and comfort measures.
  • Complete timely and accurate documentation in the EMR system.

Office Responsibilities:
  • Assist with clinical coordination and daily office operations.
  • Support scheduling, communication, and coordination with the interdisciplinary team.
  • Upload and maintain documentation within the EMR system.
  • Assist with physician orders, records, and clinical paperwork.
  • Answer clinical phone calls and provide support to patients, families, and staff.
  • Help maintain accurate patient records and regulatory documentation.
  • Support the Executive Director and Director of Nursing with daily branch operations.

What We're Looking For:
  • Current Texas LVN license.
  • Hospice, home health, or clinical nursing experience preferred.
  • Strong organizational and multitasking skills.
  • Excellent communication and customer service skills.
  • Comfortable using electronic medical record (EMR) systems.
  • Compassionate, dependable, and team-oriented.
  • Valid Texas driver's license, reliable transportation, and current auto insurance.
  • Current CPR certification.
