Hospice Marketer/Community Liaison
Location: Beaumont and the surrounding area

Are you an experienced hospice marketer who thrives on building strong referral relationships and making a difference for patients and families?

At Concho Hearts Hospice, our liaisons are trusted partners in the healthcare community. We’re looking for a motivated professional who is passionate about hospice care and enjoys developing meaningful partnerships with providers throughout the Fort Worth area.

What You’ll Do:

Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities
Grow referral sources throughout the Fort Worth service area
Provide hospice education to healthcare providers and community partners
Meet with patients and families to explain hospice services and obtain consents
Coordinate closely with the clinical team to ensure seamless admissions
Represent Concho Hearts Hospice as a trusted community resource

What We’re Looking For:

Minimum 2 years of hospice marketing experience required
Proven relationship-building and networking ability
Self-motivated with strong organizational skills
Comfortable presenting hospice education to providers and community groups
Ability to work closely with clinical teams to coordinate patient admissions
