Location: Sherman Area.

At Concho Hearts Hospice, our Nurse Practitioners focus on what matters most, providing comfort, symptom management, and support to patients and families during end-of-life care.

This PRN role offers the opportunity to work within the Sherman area, allowing for a more manageable travel radius and meaningful patient connections.

What You’ll Do:
  • Complete face-to-face visits and clinical assessments for hospice patients.
  • Evaluate hospice eligibility and recertification in collaboration with the Medical Director.
  • Provide symptom management and palliative care support.
  • Complete history & physical assessments and documentation.
  • Collaborate with the interdisciplinary hospice team.
  • Communicate clinical updates and recommendations to the Medical Director.

What We’re Looking For:
  • Licensed Nurse Practitioner (NP) in Texas.
  • PECOS certified (required for hospice documentation).
  • Minimum 1 year NP experience.
  • Hospice or palliative care experience preferred (not required).
  • Strong clinical judgment and communication skills.
  • Ability to work independently in the field.
