At Concho Hearts Hospice, our On-Call Nurses play an essential role in supporting patients and families when they need us most.

We're looking for compassionate and dependable Licensed Vocational Nurses (LVNs) and Registered Nurses (RNs)interested in joining our team serving Brazos County and surrounding communities.

If you enjoy independent nursing, have strong clinical skills, and want to make a meaningful difference while working with a supportive team, this could be a great opportunity for you!

What You'll Do:

  • Respond to after-hours patient and caregiver needs during assigned on-call shifts
  • Complete nursing visits in patients' homes, assisted living communities, and skilled nursing facilities as needed
  • Assess patient needs and provide appropriate nursing interventions within your scope of practice
  • Provide pain and symptom management in accordance with physician orders and the patient's plan of care
  • Educate patients and families on medications, comfort measures, and end-of-life care
  • Communicate changes in patient condition with physicians, RN Case Managers, and the interdisciplinary team
  • Coordinate urgent patient needs, including medications, supplies, and medical equipment
  • Provide compassionate support to patients and families during difficult situations
  • Complete accurate and timely documentation in the EMR system

What We're Looking For:

  • Active Texas LVN or RN license or qualifying Nurse Licensure Compact (NLC) license
  • Previous nursing experience in hospice, home health, long-term care, or acute care preferred
  • Hospice and on-call nursing experience is a plus!
  • Strong assessment, critical-thinking, and communication skills
  • Ability to work independently while collaborating with clinical leadership
  • Comfortable responding to patient needs during evenings, nights, weekends, or holidays as scheduled
  • Compassionate, dependable, and confident supporting patients and families
  • Current CPR certification
  • Valid driver's license, reliable transportation, and current auto insurance
  • Ability to travel throughout Brazos County and surrounding communities
