Concho Hearts Hospice is seeking a compassionate and dependable On-Call Registered Nurse to support patients and families throughout the Pilot Point, Denton, and Sherman area.
This is a dedicated on-call position with a 7-days-on / 7-days-off rotation, giving you a predictable schedule and a full week off between rotations.
Our On-Call RNs are an essential part of our clinical team, providing reassurance, symptom management, education, and skilled nursing support when patients and families need us most.
What You'll Do:
- Respond to after-hours patient and caregiver calls during your scheduled on-call rotation.
- Complete patient visits in homes and facilities as needed.
- Assess changes in patient condition and determine appropriate nursing interventions.
- Provide symptom and pain management in accordance with the patient's plan of care.
- Communicate with physicians and the interdisciplinary hospice team regarding changes in condition.
- Provide end-of-life education and emotional support to patients and caregivers.
- Coordinate medications, equipment, supplies, and other urgent patient needs when appropriate.
- Maintain accurate and timely clinical documentation in the EMR.
- Communicate important updates to the patient's RN Case Manager and clinical team.
- Provide compassionate support during changes in condition, actively dying situations, and other urgent patient needs.
What We're Looking For:
- Graduate of an accredited nursing program.
- Current Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
- Two years of RN clinical experience preferred.
- Strong assessment, critical-thinking, and clinical decision-making skills.
- Comfortable working independently while knowing when to involve the physician or clinical leadership.
- Compassionate and confident communicating with patients and families during difficult situations.
- Current CPR certification.
- Valid driver's license and current auto liability insurance.
- Reliable transportation and ability to travel throughout the assigned service area.
What to Expect:
This is a field-based hospice position. Patient visits may take place in private residences, assisted living communities, skilled nursing facilities, or other settings where our patients call home.
The position requires local travel and the ability to respond to patient needs throughout the assigned territory during the scheduled on-call rotation.
The role may involve prolonged standing and walking, assisting with patient mobility, exposure to varying home environments and weather conditions, and lifting or assisting with up to 50 lbs when necessary.
Most importantly, you'll need the ability to remain calm, compassionate, and clinically focused when patients and families need reassurance and guidance.
