At Concho Hearts Hospice, we're looking for a compassionate, dependable, and experienced PRN On-Call Registered Nurse (RN) to join our team serving The Woodlands, Texas, and surrounding communities!
Hospice care doesn't stop when the office closes, and neither does our commitment to patients and families.
Our on-call nurses are essential to that commitment, providing reassurance, symptom management, and hands-on nursing care when it matters most.
If you're a confident RN who knows the most meaningful care sometimes happens after hours, we'd love to meet you!

- Assist with end-of-life care, actively dying patients, and death visits as needed.
- Coordinate with the triage team, clinical leadership, and interdisciplinary team.
- Document all patient calls, visits, assessments, interventions, and follow-up needs accurately and in a timely manner.
- Complete required on-call reports and communicate patient updates to the daytime clinical team.
- Ensure continuity of care by providing thorough handoff communication.
- Follow hospice regulations, clinical standards, and company policies.

- Previous hospice on-call or after-hours nursing experience is a plus!

- Ability to independently assess patient needs and respond appropriately.
- Excellent communication, documentation, and time-management skills.
