Job Summary:
Primary functions include administering nursing care to patients of all ages in their place of residence, coordinating care with the interdisciplinary team, the patient, the family, and the referring agency, and assuming responsibility for coordinating care.
Responsibilities:
- Preparing and adjusting patient care plans as needed.
- Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
- Maintaining and updating medical charts, patient files, and health records.
Job Qualifications:
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), and a current driver's license.
- Experience: Two years of experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
- Skills: Nursing skills are defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Proof of current CPR. Must read, write, and comprehend English.
- Transportation: Reliable transportation. Valid and current auto liability insurance.
- Environmental and Working Conditions: Works in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
- Physical and Mental Effort: Prolonged standing and walking required, with the ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho-social needs.
