Concho Hearts Hospice is looking for a compassionate Social Worker (LMSW or LBSW) to provide PRN hospice social services in the Corpus Christi, TX area.

What You’ll Do:
- Provide emotional support and counseling to hospice patients and families.
- Assess psychosocial needs related to terminal illness and collaborate with the RN Case Manager and interdisciplinary team.
- Develop and guide individualized plans of care.
- Connect families with community and bereavement resources.
- Help patients and caregivers navigate life’s most meaningful journey with compassion and dignity.
What You’ll Need:
- LMSW or LBSW license in Texas (in good standing).
- Master’s or Bachelor’s degree in Social Work from an accredited program.
- At least 1 year of experience in long-term care or acute/medical-surgical setting (hospice experience preferred).
- Strong communication, organization, and EMR documentation skills.
- CPR certification and reliable transportation.
