At Concho Hearts Hospice, you'll have the opportunity to build meaningful relationships with your patients, provide one-on-one care, and work alongside a team that genuinely supports one another.
We're looking for a compassionate Registered Nurse Case Manager to join our Beaumont team.

What You'll Do:
  • Manage a caseload of hospice patients throughout the Beaumont area.
  • Complete comprehensive nursing assessments.
  • Develop and update individualized plans of care.
  • Coordinate care with physicians and the interdisciplinary team.
  • Educate patients and families on disease progression and symptom management.
  • Document visits using our EMR system.
  • Participate in IDT meetings.


Qualifications:
  • Texas RN License.
  • One year of nursing experience.
  • Hospice or Home Health experience preferred.
  • Strong assessment and communication skills.
  • Reliable transportation.
