Full-Time RN Case Manager – Hospice Nurse (Austin Area)

What You’ll Do:

Manage a caseload of hospice patients in the Austin area.
Provide skilled, compassionate nursing care and coordinate individualized care plans.
Complete routine visits, admissions, recerts, and IDT documentation.
Collaborate with the interdisciplinary team and participate in biweekly IDT meetings.
Communicate effectively with patients, families, and providers.
Maintain accurate, same-day charting with a provided tablet.
Deliver quality end-of-life care that reflects our values of compassion and respect.

What We’re Looking For:

Current RN license in Texas.
Hospice or home health experience preferred (training available for the right candidate).
Excellent communication, organization, and teamwork skills.
Reliable transportation and willingness to travel locally within the service area.
A passion for caring for people and making a meaningful impact every day.
