Location: Brenham, Washington County & Surrounding Areas.

We're looking for an experienced Hospice RN Case Manager to join our growing team serving Washington County and surrounding communities.

What You'll Do:
- Manage and coordinate care for an assigned caseload of hospice patients.
- Complete comprehensive nursing assessments and routine patient visits.
- Develop, implement, and update individualized plans of care.
- Provide skilled nursing care, pain management, and symptom control.
- Recognize changes in patient condition and communicate with physicians and the interdisciplinary team.
- Educate patients and families on disease progression, medications, comfort measures, and end-of-life care.
- Coordinate medications, medical equipment, supplies, and other patient needs.
- Supervise and collaborate with LVNs and Hospice Aides as appropriate.
- Participate in interdisciplinary team (IDT) meetings.
- Maintain accurate and timely clinical documentation in the EMR.
- Provide compassionate guidance and emotional support to patients and their families.
What We're Looking For:
- Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
- Previous hospice nursing experience required.
- RN Case Management experience strongly preferred.
- Strong knowledge of hospice care, symptom management, and end-of-life nursing.
- Excellent assessment, clinical judgment, and critical-thinking skills.
- Ability to independently manage a patient caseload and prioritize care needs.
- Strong communication, organization, and documentation skills.
- Experience working with an interdisciplinary hospice team.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel throughout Washington County and surrounding communities.
