At Concho Hearts Hospice, our RN Case Managers have the opportunity to build meaningful relationships with patients and families while providing compassionate, one-on-one care wherever our patients call home.

We're looking for a caring and dependable Registered Nurse (RN) Case Manager to join our team serving patients throughout Wood County.

What You'll Do:
  • Manage and coordinate care for an assigned caseload of hospice patients.
  • Complete comprehensive nursing assessments and routine patient visits.
  • Develop and update individualized plans of care.
  • Monitor symptoms and provide pain and symptom management.
  • Educate patients and families about medications, disease progression, comfort measures, and what to expect.
  • Communicate changes in patient condition with physicians and the interdisciplinary team.
  • Coordinate medications, DME, supplies, and other patient needs.
  • Participate in interdisciplinary team (IDT) meetings.
  • Provide emotional support and guidance to patients and caregivers.
  • Complete accurate and timely documentation in the EMR.
  • Collaborate with LVNs, CNAs, Social Workers, Chaplains, Physicians, and other members of the hospice team.

What We're Looking For:
  • Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
  • Strong nursing assessment and critical-thinking skills.
  • Hospice, home health, skilled nursing, or acute-care experience preferred.
  • Compassionate and confident communicating with patients and families.
  • Strong organizational and time-management skills.
  • Ability to work independently while collaborating with the interdisciplinary team.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to travel throughout the Wood County service area.

New to hospice? Hospice experience is preferred, but we're willing to support and train the right RN who has strong clinical skills and a heart for compassionate patient care.
