Full-Time RN Case Manager – Hospice Nurse (The Woodlands, Conroe, Willis)

What You’ll Do:
- Manage a caseload of hospice patients in The Woodlands, Conroe, and Willis.
- Provide skilled, compassionate nursing care and coordinate individualized care plans.
- Complete routine visits, admissions, recerts, and IDT notes.
- Collaborate with the interdisciplinary team and participate in biweekly IDT meetings.
- Communicate regularly with patients, families, and providers.
- Maintain accurate, same-day documentation with a provided tablet.
- Ensure quality end-of-life care while supporting patients and families with compassion.
What We’re Looking For:
- Current RN license in Texas.
- Hospice or home health experience preferred (training provided for the right candidate).
- Excellent communication and organizational skills.
- Reliable transportation and willingness to travel locally within The Woodlands, Conroe, and Willis.
- A heart for patients and a passion for meaningful work.
