Concho Hearts Hospice is seeking a compassionate PRN Medical Social Worker (LMSW or LBSW) to support patients and families in the Beaumont area.
Hospice social work is about more than assessments; it’s about helping families navigate one of life’s most difficult journeys with support, guidance, and compassion. Our Social Workers play a vital role in providing emotional support, connecting families with resources, and helping patients maintain dignity and comfort.

What You’ll Do:
- Assess the psychosocial needs of hospice patients and their families.
- Ability to provide compassionate emotional support to patients and families during end-of-life care.
- Assist families in identifying community resources and financial or social support services.
- Collaborate with the RN Case Manager and interdisciplinary team to develop individualized care plans.
- Participate in interdisciplinary team meetings (IDT) to support patient care.
- Assist with bereavement planning and coordinate with the Bereavement Coordinator as needed.
- Document assessments and interventions in the EMR system.
What We’re Looking For:
- Licensed Social Worker in Texas (LMSW or LBSW).
- Master’s or Bachelor’s degree in Social Work from an accredited program.
- Minimum 1 year of social work experience in healthcare, hospice, or long-term care.
- Hospice experience preferred but not required.
- Strong communication and counseling skills.
- Ability to work independently while collaborating with a care team.
- Reliable transportation and willingness to travel locally.
