Hospice On-Call LVN/RN - PRN

At Concho Hearts Hospice, our On-Call Nurses are there when patients and families need support the most. Whether providing symptom management, answering urgent questions, or offering reassurance during difficult moments, our nurses play a vital role in delivering compassionate end-of-life care.
We are seeking a dedicated LVN or RN to join our Austin team and provide after-hours support to hospice patients and families.
What You'll Do:
- Respond to after-hours patient and family calls with professionalism and compassion.
- Assess patient needs and provide appropriate nursing interventions.
- Complete urgent visits in patients' homes or facilities as needed.
- Provide symptom management and end-of-life support.
- Communicate patient status changes with the clinical team and physicians.
- Educate and support patients, caregivers, and families.
- Complete timely and accurate documentation in the EMR system.
What We're Looking For:
- Active Texas LVN or RN License.
- Hospice, home health, long-term care, or acute care experience preferred.
- Strong assessment and critical thinking skills.
- Ability to work independently and make sound clinical decisions.
- Excellent communication and customer service skills.
- Reliable transportation and willingness to travel throughout the service area.
- Current CPR certification.
Hospice experience is preferred but not required. We are willing to train the right nurse who has a passion for compassionate patient care.
