Concho Hearts Hospice PRN On-Call RN 7 days on / 7 days off On-Call cycle
Area: Pilot Point / Denton / Sherman area

Job Summary:
This is an on-call-only position; it will be a cycle of 7 days on call and 7 days off call. It is a full-time position. Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency; and assume the responsibility for coordination of care.

RN Responsibilities:

Preparing and adjusting patient care plans as needed.
Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
Maintaining and updating medical charts, patient files, and health records.

RN Job Qualifications:

Education:
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure:
- Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Driver's License.
Experience:
- Two years of experience as a Registered Nurse in a clinical care setting preferred; hospice preferred.
Skills:
- Nursing skills as defined by generally accepted standards of practice.
- Good interpersonal skills.
- Specialized skills preferred in palliation and end-of-life.
- Proof of current CPR. Must read, write, and comprehend English.
Transportation:
- Reliable transportation.
- Valid and current auto liability insurance.
Environmental and Working Conditions:
- Works in patients' homes in various conditions, with possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
Physical and Mental Effort:
- Prolonged standing and walking required, with the ability to lift up to 50 lbs and move patients.
- Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho-social needs.

Transportation:

Reliable transportation.
Valid and current auto liability insurance.
